Dear Sir/Madam

I refer to the <description of service> that you carried
out for me on <date>.  On <date> I noticed that the work
was defective, only <number> of days after you completed
it.
The defects in the work:

It was implied in the contract between us that you would
carry out the work with reasonable skill and care, and 
would use materials of reasonable quality.  This is laid
down by the Supply of Goods and Services Act 1982 [NOT
SCOTLAND]
The defects show that you have failed to fulfill these
obligations and I have a claim against you for breach of
contract.

However, while reserving my right to claim against you,
I will allow you an opportunity to rectify these
defects within 14 days at no cost to me.

If you do not carry out this work within the specified
time I shall take steps to engage another contractor to
put the matter right and recharge the work to you, if
necessary through legal means.
I look forward to hearing from you.
